All rented equipment must be insured by the Customer, and a current certificate of insurance is required to provided as evidence of coverage. The policy should insure the rented equipment in an amount no less than the full replacement value of said equipment, and also provide Commercial General Liability in the amount of at least $1,000,000 per occurrence and $2,000,000 per aggregate. The insurance certificate should list Machado Visuals LLC as Certificate Holder, Additional Insured (with respect to general liability), and Loss Payee (with respect to loss or damage to rented equipment):

Additional Requirements
The policy must provide specific coverage for miscellaneous rented equipment, which should be indicated in the “Other” section of the certificate. The deductible must be clearly stated and not exceed $2,500. The insurance must be written on a worldwide replacement cost basis and cover all risks.
Unattended automobile exclusions are not acceptable. The policy must include coverage for equipment while in transit.
Equipment rentals are COD only. Invoices must be paid in full prior to picking up equipment.
